Use this form for each additional child joining a private group session or a paired / small-group tutoring session, or if Andy has asked you to complete a registration form. I understand that children under 8 need a parent/guardian (or named responsible adult) on site, and that an adult stays with every child on the beach.

I understand that the final decision on safety, weather and whether a session goes ahead is Andy’s.

I consent to STEM by the Sea holding the health, allergy and SEND information above, to keep my child safe. I can withdraw this consent at any time.
